Consent Form In
compliance with Bill A592,
Township of Franklin Schools shall
not disclose any personally identifiable information about a student
without receiving prior written consent from the student's parent or
guardian.

form for approval to post personally identifiable information about
a student is provided below.
Frequently, student art work is
posted for use with teacher web pages. In order to acknowledge
student's artwork by giving first and last name, the second box on
the form must be checked. The wording is:
I/We GRANT permission for this student's
photo/image and name to be published on the school and/or
district's public Internet site.
